Ice time and personal equipment are the participant's responsibility unless otherwise noted. S13 Hockey is not liable for injuries sustained during voluntary athletic activity, except where caused by our negligence.
S13 Hockey may photograph or film sessions for promotional use on this site or our social channels, unless a guardian opts out in writing to [email protected].

S13 Hockey (Jason and Taryn Swit), Skills, Training Sessions and Scrimmages.
We (Participant and Participant's Parent/Guardian) hereby acknowledge that we are voluntarily participating in the S13 Hockey Sessions, inclusive of but not limited to Skills, Training, and Scrimmage Sessions (hereinafter referred to as "the Sessions"). We understand that participation in the Sessions involves certain risks, including but not limited to physical injury. In consideration of being allowed to participate in the Sessions, we agree to the following:
I am aware that participating in hockey activities, including but not limited to skating, puck handling, and physical contact, carries inherent risks of injury. I voluntarily assume all such risks.
Although coaches will be guiding players through off-ice and on-ice instruction, this is NOT an Ontario Licensed day camp or child care service. I understand that parents/guardians are responsible for their player at all times.
I hereby release, waive, discharge, and covenant not to sue the organizers of the Sessions, including all volunteers and instructors (collectively referred to as "the Released Parties"), from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant or any property belonging to the participant, whether caused by the negligence of the Released Parties or otherwise, while participating in the Sessions.
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my participation in the Sessions, whether caused by my negligence or otherwise.
In the event of an injury or illness during my participation in the Sessions, I authorize the Sessions staff to seek and consent to any emergency medical treatment deemed necessary for the participant's well-being.
By participating in the S13 Hockey Skills and Training Sessions, I grant the organizers the right to take photographs and videos of me during my participation and to use such materials for promotional purposes without compensation.
I agree to comply with all rules, regulations, and instructions of the Sessions organizers and staff.
